Special Focus Facility

Special Focus Facilities are selected by the state survey and certification agencies from a group of facilities with high Special Focus Facility (SFF) priority scores provided by CMS (usually 15 facilities). The calculation of the SFF priority scores follows algorithms described in a CMS memorandum. PointRight has implemented this algorithm.

The SFF Report shows the SFF priority scores and their percentile ranks in the state for all of the facilities in your organization. It presents the scores, shows how they were calculated, and provides additional insight into the meaning of the scores.

The SFF Report offers analysis useful in responding to SFF scores.

  1. The facility’s SFF score (and percentile rank) if complaints were counted over 36 months rather than over three Survey Cycles – and whether the facility’s score and rank would improve if this were done
  2. Would SFF Percentile improve with survey district benchmarking?
  3. The facility’s SFF percentile rank if benchmarking was done over the survey district rather than the entire state – and whether the facility’s rank would improve if this were done
